Pattern of Retinopathy: Although the locus of toxic damage is parafoveal in many eyes, Asian patients often show an extramacular pattern of damage. Dose: We recommend a maximum daily HCQ use of 5.0 mg/kg real weight, which correlates better with risk than ideal weight.

Recommendations on Screening for Chloroquine and Hydroxychloroquine Retinopathy Retinal toxicity from chloroquine CQ and its analogue, hydroxychloroquine HCQ, has been recognized for many years. Chloroquine toxicity remains a problem in many parts of the world, but is seen less frequently in the United States where the drug largely has been replaced by HCQ. To properly screen patients for Plaquenil toxicity, you should have SD-OCT and automated visual fields; it is also beneficial to include FAF and mfERG. Although the latest guidelines suggest that Amsler grid, color vision and fundus photography are not considered necessary for screening patients on Plaquenil, they can still provide useful information. Dosage levels that can cause toxicity 6.5 mg/kg Hydroxychloroquine Plaquenil19 3 mg/kg Chloroquine Arlen With Plaquenil Plaquenil is manufactured in only a 200 mg tablet The typical dosage is either 200 or 400 mg per day 200 mg daily puts anyone under 68 pounds at risk1

Risk of Toxicity: The risk of toxicity is dependent on daily dose and duration of use. There are no similar demographic data for CQ, but dose comparisons in older literature suggest using 2.3 mg/kg real weight.

  7. In analyzing SD-OCT, it is imperative to pay special attention to the outer retina Figure 3. Thinning of the photoreceptor layer, ellipsoid layer, outer nuclear layer, and RPE can be among the earliest signs of hydroxychloroquine toxicity. 4 These changes often occur in the parafoveal area, although they can be more peripheral in Asian patients.

SD-OCT is an objective test for toxicity that complements 10-2 visual field testing. 10-2 visual testing is probably more sensitive, but is heir to interpretational difficulties and variability not so much a problem with SD-OCT. Joint pain & Sjogrens? Plaquenil? DailyStrength Sjs causes an inflammatory arthritis which can cause swelling and pain but does not do harm to the joints themselves. I guess you could call that a blessing. The difference between Plaquenil and drugs like methotrexate is the P blocks inflammation only but the M and I am not sure of this attacks the white blood cells.
